Application, Commissioner's Award for Excellence or Commissioner's Award for Excellence Fill-in format
Does your unit meet the requirements for this award developed by the Council Commissioner Staff?  This form should be filled out and send with your rechartering paperwork whether your unit meets the requirements or not.

Application, Council Canoe Rental Agreement 
Rental agreement and conditions of use for canoes from our facilities.

Application, Counselor for Merit Badges
Fill out this form to become a merit badge counselor.  Send in to the Council Office along with a BSA Adult Registration Form, we will email you your registration number, used to sign-in for Youth Protection Certification Training.  (training available on line)

Application, Local Tour Permit Application
Print on Legal Size Paper (8.5" x 14"). This application must be filed with council service center two weeks in advance of scheduled activity for proper clearance.  It is used for trips of less than 500 miles outside the Finger Lakes Council boundaries.

Application, National Tour Permit Application 

Print on Legal Size Paper (8.5" x 14"). A National Tour Permit is required for all groups traveling to areas 500 miles or more one way from home area (local council camp excepted), or crossing national boundaries into the territory of other nations. This application should be submitted, typed or printed, to the council service center for approval at least one month before your tour.  Addendum to the National Tour Permit  will be needed to list extra informaiton.

Application, Transfer
Moving from one Unit to another?  This form should be filled out especially if you are traveling out of the area.  Form to be filled out by your former leader to give to your new leader.  For both youth and adult members.

Application, Veterans
Adult recognition for years registered.  After 5 years of registered service in BSA (time as a youth also counts) submit application to the council office.  This recognition is usually handed out yearly at the District Recognition Dinner held in March.

A fun activity held in February for Units and Scouts to earn money for camp and/or to enrich their Unit treasuries

Form, Accident & Sickness Claim
The Finger Lakes Council provides Accident & Sickness Insurance coverage for all registered adults and youth.  In the event of an incident a claim needs to be filed with the office in order to take care of any unpaid medical bills.  For details read the insurance brochure that is provided in your unit's rechartering packet each year.

Form, Illness - Close Call - Accident Report
The Health & Safety/Risk Management Committee developed this report to generate a history of things to watch out for and to make our units more safe.  Fill out this form whenever: 1) A medical problem has had a significant effect on an individual’s participation, 2) An individual receives medical attention, 3) An incident occurs which does not result in injury but reveals a potential safety problem (a close call)

Use this form to report any inappropriate behavior, by youth or adults towards others or property and then submit it to the council service center, care of Health & Safety/Risk Management Chairperson (examples include – biting, hitting, malicious activities)

Medical Form,  Adult Health Survey & Waiver of Liability
If an adult is attending Camp this summer for 72 hours or less you can file this form in place of a full doctor's physical

Medical Form,  FLC's Class 3 Health & Medical  for youth and adults
Youth:  To be filled out by parent/guardian (left column) & doctor (right column and backside).  Must be turned in to attend any Cub Scout or Boy Scout resident camp.  Note: A physical is good for three (3) years with the parents' information updated each year.
Adults:  Physical is needed for all resident camps and is good for three (3) years up to the age of 40, after which a physical is needed every year.

Every Scout should fill out one of these forms to be kept on file by the immediate Scout Leader and/or taken on every outing to make available needed information in the event of an emergency.

NEED TRAINING!!  LEADER FAST START, LEADER YOUTH PROTECTION, ETC:  These training courses are available online with new courses being added all the time through MyScouting on our training homepage

Adult Leader Training Award Progress Criteria
A trained leader is knowledgeable and confident in the role being performed.  BSA has a recognition program for adults that go through the steps to becoming trained.  Track your progress then submit to the council office for presentation.  Usually awards are presented at each district's annual recognition dinner held in March. 

Training, Youth Protection Resources
Because of the great concern with child abuse in our society Youth Protection training has been developed to safeguard both our youth and our adult members.  The following is published materials that go along with audiovisual materials that can be requested at the council service center.

Youthpower Dinner
Each year the Finger Lakes Council holds an event to celebrate distinguished citizens of our area.  This prime rib dinner includes a silent auction of various items and services from around the four county area.
